Insurance/Forms of Payment

Beth tries to keep insurance billing simple.  She will verify benefits prior to service delivery and then submit to insurance for reimbursement on the family's behalf.  She is currently a preferred provider with Blue Cross Blue Shield and Humana.

Please contact Beth to learn about her competitive private pay rates for families who do not have/choose not to use insurance benefits.

If the family prefers to pursue Beth's services through Illinois's Early Intervention Program, all billing will be efficiently managed/submitted by Clearbrook's CHILD Therapy Program.
